The following forms are provided for endorsed/approved Southern Baptist chaplains and counselors in ministry.  The form file(s) should be downloaded to your computer (saved/opened on your computer).  The completed form can be returned to our office via email (chaplainreport@namb.net) or sent via regular mail.  The form files are in Microsoft Word (.doc) format unless otherwise indicated.

We recommend that you save a blank form and then save another copy of the file with your information (address/endorsement type) completed.  This process will help expedite your completion of the form each quarter.   All chaplains and counselors in ministry who provide an email address will receive a reminder via email just prior to the end of each quarter about the report submission.
