The following form is provided for endorsed/approved Southern Baptist chaplains and counselors in ministry.  The form file should be downloaded to your computer (saved/opened on your computer).  The completed form can be returned to our office in one of three ways:

1.  via email (chaplainquarterly@namb.net)

2.  sent via regular mail to Chaplaincy Quarterly, North American Mission Board, 4200 North Point Parkway, Alpharetta, GA 30022,

3.  or, fax to 678-624-3611. 

Note:  Some computer networks do not allow attachments on email, i.e. military networks overseas, when this occurs print the form and send via mail or fax.  You could also use a non-secure email program, like yahoo or hotmail, to attach and forward the form.

The form file is in Microsoft Word (.doc) format unless otherwise indicated.

To download, complete and then send report as email attachment: 1. right click on the link and 2.select "Save Target As," then save to your hard drive.

We recommend that you save a blank form and then save another copy of the file with your information (address/endorsement type) completed.  This process will help expedite your completion of the form each quarter.   All chaplains and counselors in ministry who provide an email address will receive a reminder via email just prior to the end of each quarter about the report submission.
